2. The membership is provisional and will be regulated as per the rules of ‘Jeevothan Foundation’.

3. The membership contribution [Rs 100 /- only per year]will not be refunded in case of termination of membership before one year. The ID-card of Jeevothan will be given only after the deposit of membership contribution.

4. The membership is completely voluntary in nature.

5. Member has to submit Id proof, address proof and two recent passport size photographs along with this form.
